Know what changed. Know what did not.

Evidence disciplines the conclusion.

Keep knowledge status visible

Observed means seen or measured directly. Inferred means a reasoned conclusion from observations. Hypothesis means not yet tested. Contradiction identifies a conflict between accounts or between promise and practice. Unknown means there is not enough information. These statuses describe knowledge, not marketing confidence.

Activity and outcome are different

Complete is not delivered. Delivered is not accepted. Implemented is not outcome. An observed outcome is not automatically attributed impact. Workshops and meetings show activity; they do not alone establish transformation. State what the evidence actually supports.

Publish a claim that can be inspected

Every result requires institution, result, period and status, with a source and agreed publication permission. Separate institutional statements, direct observation and independent support. Keep contradictions and limitations beside the specific claim. Missing evidence is not proof of failure.

Return to the baseline

Agree what will be observed again, for whom, in which conditions and after which interval. Record changes in workload, staffing or measurement. A credible account can show improvement, no movement or an inconclusive comparison.
