Trace the visible symptom to the institutional condition producing it.

The strategy is clear. The field runs differently.

Compare the intended journey with the actual next step. Where do rules become ambiguous, approvals multiply or local workarounds become the operating system? Follow a specific instance before naming a culture problem.

The service works. The experience does not.

An institution may complete the technical task while leaving a person uncertain, unheard or passed between desks. Examine explanation, waiting, privacy, continuity and responsibility for the next step. Functional delivery and an understandable experience are different questions.

Everyone approved. Nothing moved.

A slow path may be an authority problem rather than a people problem. Record which decision is waiting, who has the right to make it, which information is missing and what happens after approval. More reminders do not resolve an undefined decision path.

The work is strong. Its support is weak.

A claim can be true yet hard to inspect. Connect the result to its source, period, status and scope. Conversely, repeated positive messages do not establish consistent practice. Investigate the gap before choosing either communication or operational change.

Training ends. Practice stays the same.

Check whether people have the authority, tools and feedback to use what they learned. Observe a specific behavior in work, test one change and review it with its owner. Attendance is an activity measure; the question is what the next team does differently.
