Decision record
Keep the reasoning, responsibility and review conditions of a decision together, so the next team can act without reconstructing the meeting.
By Roadyom Editorial Team. An original working template, not a validated assessment instrument or client case.
When and how to use it
Use this during a decision review or immediately after a decision. Ask the decision owner to confirm the chosen option, then agree what evidence would justify revisiting it. A completed record is a record of a decision, not proof that it worked.
The working record
- Decision question and scope
- Institutional intent and affected journey
- Options considered and trade-offs
- Evidence available, sources and gaps
- Chosen option and rationale
- Decision owner and delivery roles
- Dependencies and next action
- Expected observable change
- Review date and reconsideration conditions
- What was observed at review; confidence and limits
