Trust is often lost between two completed steps.

A finished service can still leave a person without an understandable next step.

Look at the transition

A desk may finish its task correctly and the next desk may also work correctly. The gap can sit between them: no explanation, no accepted handoff and no visible owner. A person experiences the whole journey rather than departmental completion. Begin by following one transition from the moment the first team considers its work finished to the moment the receiving team accepts responsibility. Record the instructions given, the information transferred and what the person understood. This reveals a condition that a departmental activity report may never show.

Separate explanations before intervening

Uncertainty may come from unclear language, missing authority or an unavailable receiving team. These explanations call for different changes. Compare the person’s account, direct observation and the operating rule; retain cases where the transition worked. Ask what was different in those cases. Avoid turning every problem into communication training. If nobody owns acceptance, better wording cannot create ownership. If the path is clear but the explanation is confusing, restructuring the institution may be unnecessary.

Test a complete handoff

Define the receiving owner, the minimum information and a short explanation of the next step. Try the revised transition in an agreed scope, including busy periods and exceptions. Re-observe whether people know where to go and whether the receiving team accepts the handoff. Compare like conditions and keep unanswered cases. A form marked complete establishes that a record was made; it does not establish that the person experienced continuity. Leave the review routine with the teams responsible for both sides of the transition.
