Turn experience into institutional memory.
A system that cannot learn will drift.

Retain the reason, not only the decision
Store the question, alternatives, evidence available at the time, decision owner and review conditions. Keep failed tests and their context. A record should help the next team understand when an old decision still applies and when it needs to be reconsidered.
Observe, decide, act, re-observe, learn
Learning is a loop within the system. Link a field observation to a bounded decision, the action taken and comparable follow-up evidence. Date the knowledge and record its status so an old observation does not become a permanent truth.
Update the playbook with ownership
A proposed revision names the recurring friction, supporting sources, expected behavior and test conditions. The responsible owner reviews the proposal, runs the test and accepts or rejects the change. Retain the prior version and the reason for revision.
